New chicken lady needs help!

Good afternoon all! I'm new to this chicken "stuff" and someone directed me here. This site is pretty amazing! Today I went out to see my chickens and one of my ladies is all puffed up and not moving very fast. She's got a chalky yellow discharge from her vent - not a lot but something I haven't seen before. All my girls were hatched this spring so they're fairly young. Any one have ideas on what I need to do? Thanks in advance!

Sometimes yellow poop 'can' mean worms. Have your chickens been wormed? Also, does her crop feel normal size in the morning before she eats?
